Sinus of Valsalva (SOV) and Aortic Root Assessment

The sinus of Valsalva diameter (SOVd) is a critical measurement in evaluating aortic root morphology. Age-stratified formulas help determine expected normal values based on body surface area (BSA), aiding in the diagnosis of aortic root dilation.

Key Formulas

  • BSA Calculation (Mosteller formula): [ \text{BSA (m²)} = \sqrt{\frac{\text{Height (cm)} \times \text{Weight (kg)}}{3600}} ]

  • SOVd Formulas:

  • <20 years: ( \text{SOVd} = 1.02 + 0.98 \times \text{BSA} )

  • 20-39 years: ( \text{SOVd} = 0.97 + 1.12 \times \text{BSA} )

  • ≥40 years: ( \text{SOVd} = 1.92 + 0.74 \times \text{BSA} )

Definition of aortic aneurysm

  • Sinus of Valsalva >45 mm.

  • Any other segment >40 mm in diameter.

*Risk factors: previous dissection, family history of dissection, severe aortic regurgitation or mitral regurgitation, planning pregnancy, uncontrolled hypertension or annual growth >3 mm.

Limitations

  • Formulas assume normal body habitus

  • Ethnic variations may require adjusted thresholds

  • Always correlate with imaging measurements

  • Not valid for pediatric patients <15 years
